80 year old female with groin mass

This is a difficult case and I would like to hear some opinions.


This is a 80 year old female with previous history of invasive ductal carcinoma, moderately differentiated (Nottingham; 6) , status post mastectomy. The mastectomy was done 3 years ago and she recovered completely after the surgery. She is not taking any chemo..or hormonal therapies. Now she presents with and enlarged groin mass, with initial impression of an enlarged lymph node. The mass is 2.6cm well circumscribed and somewhat lobulated cut surface. Areas of trapped mature adipose tissue were obvious. There is no gross or microscopic necrosis, no mitotic figures, no apoptosis or any nuclear atypia. The lesion has thin capsule.

Immunostains:

CD34...POSITIVE
DESMIN...NEGATIVE
SMA...........NEGATIVE
ER.............NEGATIVE
PR.............NEGATIVE
