40 yo female upper GI endoscopy
40 year old female.
This is a 40 year old female who underwent a upper GI endoscopy. Initially she had some vague upper GI troubles with mild pain and reflux. She tested positive for H. pilory however she failed to improve after therapy. On endoscopy there was an ulcer located on the lesser curvature of the stomach. As a rule I automatically get 3 deeper levels on all stomach ulcers and in this case it did show more of the problem shown bellow. It is amazing how something so small and innocent looking can actually kill.

Diagnosis: Poorly differentiated adenocarcinoma with signet ring cells, diffuse type.
Use the images to train your eyes to recognize the signet ring cells hiding between the glands.
Notice also that there is a intestinal metaplasia adjacent to the carcinoma. Compare the goblet cells with the signet ring cells.
This case has some good news for her since the tumor is located only whitin the mucosa (T1) and negative lymph nodes.
